Podstawa prawna zbierania tych danych jest art. 6 ust. 1 pkt. b Og\u00f3lnego Rozporz\u0105dzenia o Ochronie Danych Osobowych z dnia 27 kwietnia 2016 r.<\/p><p>3. W trakcie rejestracji w naszym gabinecie prosimy o nast\u0119puj\u0105ce dane osobowe: imi\u0119 i nazwisko; adres (ulica, numer domu\/numer domu i lokalu, kod pocztowy i miasto); numer telefonu; adres mailowy.<\/p><p>4.
